What is Supported Decision-Making (SDM)? SDM is a tool that allows people with disabilities to retain their decision-making rights by identifying supporters to help make decisions in lieu of legal guardianship.
What does Supported Decision-Making (SDM) actually look like? SDM will look different for everyone. SDM allows a person to identify supporter(s) to help understand, make, and communicate their choices by executing a Supported Decision-Making agreement. The document helps third parties such as doctors, lawyers, or banks understand and accept decisions of the person with a disability.
